JPHSA SERVICES:
Adult Community Support Services
The Adult Community Support Division focuses its efforts on providing community-based assistance to adults with a serious mental illness and/or addictive disorder. Community Support staff members provide both direct services and facilitate referrals to other agencies.
Crisis Intervention
Community Support provides 24-hour mobile crisis intervention for children and adults with mental illness, an addictive disorder, and/or a developmental disability. The Mobile Crisis Team is dispatched during emergency situations for individuals living in Jefferson Parish.
In addition to rapid response, the Mobile Crisis Team provides emotional support, in-home stabilization (when possible), referral to ongoing support programs, and follow up to assure continuity of care. Call 504-832-5123.
Medicaid Enrollment
Community Support provides Medicaid Enrollment assistance to individuals receiving services through JPHSA. Call 504-838-5700 for locations and hours of operation.
Referred Services
With appropriate referral from a provider and criteria-based pre-authorization, Community Support works to match adults with the services they need.
- Case Management
- Homeless programs (transitional & permanent)
- Housing services coordination
- Assertive Community Treatment for adults with severe mental illness (clinic without walls)
- Respite care
- Employment and vocational services coordination
- Residential programs for individuals with substance abuse issues
