JPHSA SERVICES:
Adult Community Support Services

The Adult Community Support Division focuses its efforts on providing community-based assistance to adults with a serious mental illness and/or addictive disorder.  Community Support staff members provide both direct services and facilitate referrals to other agencies.

Crisis Intervention

Community Support provides 24-hour mobile crisis intervention for children and adults with mental illness, an addictive disorder, and/or a developmental disability.  The Mobile Crisis Team is dispatched during emergency situations for individuals living in Jefferson Parish.

In addition to rapid response, the Mobile Crisis Team provides emotional support, in-home stabilization (when possible), referral to ongoing support programs, and follow up to assure continuity of care. Call 504-832-5123.

Medicaid Enrollment

Community Support provides Medicaid Enrollment assistance to individuals receiving services through JPHSA. Call 504-838-5700 for locations and hours of operation.

Referred Services

With appropriate referral from a provider and criteria-based pre-authorization, Community Support works to match adults with the services they need.

• Faith-based outpatient addictive disorders treatment

Louisiana Permanent Supportive Housing (PSH)

The Louisiana Permanent Supportive Housing Program provides housing to persons with disabilities who have low incomes and need support in order to maintain housing and not become homeless or institutionalized. In Jefferson Parish, the Jefferson Parish Human Services Authority (JPHSA) has been designated as the Local Lead Agency responsible for operating the program.  There are 330 PSH units throughout the Parish.

PSH are special rental apartments that come with supports, provided by a team of professionals, for people who have difficulty living independently and successfully in the community and may become homeless or institutionalized without the supports. These housing supports include things like reminders to pay the rent on time, learn how to budget money, learn the skills needed to keep an apartment clean and maintain personal hygiene as well as help arranging medical appointments or other support services. Only people with disabilities who need these types of support are eligible for PSH.

To be eligible for the housing and supports available through the program, the household must meet three eligibility criteria:

1) a member of the household must have a substantial, long-term disability;

2) The household income must be at or below the following:

*Family of 1 - $21,700/per year
*Family of 2 - $24,800/per year
*Family of 3 - $27,900/per year
*Family of 4 - $30,950/per year
*Family of 5 - $33,450/per year
*Family of 6 - $35,950/per year
*Family of 7 - $38,400/per year
*Family of 8 - $40,900/per year

3) and, because of the disability, the household must be in need of supportive services in order to live independently and successfully in the community.

Applications are accepted during an open application period and households are selected through a lottery process.
